Chronic Illness Therapy

Your body has changed. You don’t have to lose yourself too. Specialized virtual psychotherapy with Jacqueline On, MSW, RSW, supporting adults across Ontario in navigating the profound emotional, physical, and identity shifts of chronic health conditions.

When Chronic Illness Takes Over Your Life

Living with a chronic condition impacts far more than your physical body; it shapes how you navigate daily routines, energy fluctuations, and your peace of mind.

You might recognize yourself thinking:

I’m so exhausted, but rest doesn’t fix it.

You carry an unrelenting, heavy fatigue that leaves you drained even after a full night’s sleep.

I’m just so tired of being in constant pain.

Living with ongoing physical discomfort keeps your body on high alert, leaving you wishing for even a brief moment of relief.

I dread going to my next medical appointment.

Your stomach knots up days in advance as you worry about being dismissed, misunderstood, or forced to start over with a new provider.

I feel like no one truly understands what this is like.

You carry the quiet, invisible isolation of managing an unpredictable body in a world that expects you to function as normal.

I’m stuck on an endless energy rollercoaster.

You overdo it on days when you feel okay, only to crash for days afterward when your body forces you to stop.

What is Chronic Illness Therapy?

When you live with a chronic condition, your medical team focuses on your body. You may have tried various medications, had multiple sets of bloodwork and scans done, and attended endless appointments trying to find solutions. Maybe you’ve even had to fight to be taken seriously. Maybe your suffering feels invisible to those around you.

Rarely is there a space dedicated to what this condition is doing to your heart, relationships, identity, and mind.

Chronic illness therapy is the bridge between your medical reality and your mental well-being. Living with a long-term condition comes with a heavy emotional load: grief for who you used to be or what you were able to do, the frustration of an unpredictable body, and the isolation of living in a judgmental world that tends to misunderstand.

This therapy is not a physical treatment, nor is it a promise of a cure. I will never be able to guarantee the resolution of your condition or rid your body of pain.

Instead, this is a specialized, trauma-informed virtual space dedicated to helping you process the emotional and nervous system toll of living in an unpredictable body.

How We Work Together

Our sessions are designed to support the invisible emotional and physical weight you carry every day. Depending on where your energy is at, our time together generally focuses on three areas:

Processing Chronic Sorrow and Identity Shifts

Chronic illness often brings with it a recurring grief when hopes and expectations don’t match your daily reality. Therapy sessions offer a dedicated space to unpack and honour the valid grief and frustration of living with a long-term condition.

You are allowed to mourn:

  • The life and future you had originally planned
  • The career, routines, or hobbies that had to change
  • The pre-illness version of yourself

Together, we will work on accepting your current physical reality without giving up on your life. By separating your sense of self from what your body is experiencing, you can build a full, meaningful life alongside your condition. You are so much more than your illness.

Soothe a Hyper Vigilant Nervous System

Living in constant discomfort or anticipating the next flare-up keeps your body in survival mode. This chronic stress does more than weigh on your mind; it can amplify your physical symptoms, increase pain signals, and heighten suffering.

In our sessions, we will use gentle, body-based somatic tools to send safety signals back to your brain. These tools help to calm your nervous system and slowly ease the physical tension you’ve been holding onto.

Pacing and Reclaiming Agency

It’s easy to get caught in an endless “push-and-crash” cycle: overdoing it on good days, only to pay for it later when your body forces complete rest.

In therapy, we will work on practical pacing strategies so you can finally stop riding that energy rollercoaster.

Together, we will:

  • Learn to set boundaries with others without the heavy layer of guilt
  • Actively hunt for glimmers (i.e., small, meaningful micro-moments of safety, joy, and purpose)

Our goal is to help you reconnect with your true self, so you can live more of your life on your own terms.

Dedicated Virtual Therapy for Your Changing Body

Living with a long-term health condition can feel like an isolating, uphill battle, but you don’t need to face this alone. In our sessions, we will attune to where your body is at on any given day, adjusting our pace to match your current capacity.

Our sessions are held online via secure video, allowing you to join from your couch, your bed, or wherever you feel most comfortable. By eliminating travel time, sensory-heavy waiting rooms, and extra physical effort, you can save your energy for what truly matters. This is your virtual space to rest, process the weight of your condition, and rebuild a gentle relationship with your body.

What This Therapy Is, and What it Is Not

Before we begin, I want to be entirely transparent with you about what this space is, and what it is not.

  • This is not a promise of a cure. I will never offer you a blank-cheque guarantee to medically resolve your condition or rid your body of pain. You have likely already tried countless treatments, and you do not need another person promising a quick fix.
  • This is a dedicated, safe space to process the complex emotional, physical, and identity changes that come with a long-term health condition. Together, we will honor the chronic sorrow that often accompanies these diagnoses—the ongoing grief for the life you planned, alongside the reality of the life you are living.
  • This is a space to explore the deep connection between your mind and body. Chronic symptoms and persistent pain are often intimately linked to stress, trauma, or unexpressed emotions that have become stuck in the physical body. Therapy gives us a way to safely listen to what your body is holding and release it.
  • This is a space to help you retrain your nervous system. When you live with a long-term condition, your body can get trapped in a constant state of high alert or survival mode. We use therapy to communicate safety back to your system, helping to dial down the distress and physical amplification of your symptoms – an ongoing process over time.
  • This is an active collaboration to help you find sustainable, self-compassionate ways to care for yourself, rebuild your relationship with your body, and reclaim a sense of agency. We will work on identifying small, meaningful ways you can still connect to joy, purpose, and vitality, even on your highest-symptom days.

How We Focus Our Sessions Together

While chronic illness and chronic pain often walk hand in hand, the topics you bring to each session might look quite different depending on what your body and mind need that day. In our sessions, we will adapt our focus to meet your specific needs. Your nervous system will lead the way.

  • Navigating Chronic Illness: When your illness takes center stage, we may explore the profound identity shifts, the daily fatigue, the frustration of unpredictable flare-ups, and the emotional toll of navigating a complicated medical system. Together we may look at how to nurture genuine hope and rediscover a sense of purpose, helping you cultivate a life that still feels expansive and uniquely yours alongside your diagnosis.
  • Working Directly with Chronic Pain: When pain dominates your day, we may turn our attention directly to the physical sensation itself. Pain is a survival signal from a highly protective nervous system. Using gentle, targeted tools like Emotional Freedom Techniques, we will work to dial down the threat response, untangle the emotional distress under the physical discomfort, and help your body remember how to find pockets of safety and ease.

How Chronic Illness & Chronic Pain Therapy Can Help

While managing a chronic condition can feel very unpredictable and often out of our control, therapy can fundamentally shift how you experience and live alongside it. Together, we can work to:

  • Unpack and honour the valid grief and frustration of living with chronic illness.
  • Learn pacing so you can stop riding the exhausting “push-and-crash” cycle.
  • Calm the hyper-vigilant nervous system response that amplifies pain signals.
  • Separate who you are from what your body is experiencing, reclaiming your identity.

Common Symptoms Addressed

  • Chronic or recurring physical discomfort and pain that disrupts daily functioning.
  • Persistent fatigue and depleted daily energy reserves.
  • Health-related anxiety or hyper-vigilance regarding medical appointments and symptom flares.
  • Deep grief or sadness over the sudden loss of your pre-diagnosis lifestyle and physical abilities.
  • Social isolation or feeling deeply misunderstood by friends, family, or coworkers regarding your health.
  • Disrupted sleep patterns caused directly by physical symptoms or stress.

My Approach

I blend emotional processing with gentle, body-based modalities so that our work together feels safe and supportive to your nervous system. Our session focus will center around what your body needs on any given day.

  • Acceptance and Commitment Therapy (ACT): We will work on making space for difficult thoughts and the reality of your symptoms, helping you anchor back to what still brings meaning, vitality, and purpose to your life.
  • EFT Tapping and Somatic Tools: Chronic symptoms often involve a highly protective nervous system. We will use gentle acupressure tapping to lower stress hormones and safely process the stored frustration, fear, or anger that you may have been holding onto.
  • IFS-Informed Parts Work: We will listen to internal conflict that often comes with illness – honouring both the fierce part that pushes you through fatigue to avoid judgment, and the vulnerable part that carries guilt when your body demands to rest.

Ultimately, our time together is completely for you. Whether today is a high-symptom day that requires adjustment, or a day that feels spacious enough for deeper processing, we will meet your body where it is.
